Caring for a loved one with Alzheimer’s disease can be challenging. At Connecticut Nursing Services, we understand the physical and emotional challenges that come with caring for a person with cognitive changes or impairment.
We understand the first few days at home following discharge are most critical to ensure healing and wellness. To ensure a successful transition from the hospital or rehab to home setting, Connectiucut Nursing Services will assess the needs of the client and provide an individualized plan of care.
Our Care Coordinator responsibilities include ensuring the daily well-being of our clients, serving as a liaison to families no matter where they are located and making sure the care plan is being followed. Our Care Coordinator also alert families to any problems and play a vital role in facilitating, coordinating and attending appointments with the necessary healthcare providers.
